The approach to peroneal tendinopathy is twofold

Peroneal tendinopathy is not a common problem in runners, but when it does happen it can be frustrating to manage. By like any overuse injury, the principle of management is actually quite simple: load management. You have to reduce the load to a level below what he tissues can tolerate. So to decrease the load, you reduce activity levels use lateral wedging so the muscles does not have to work so hard. To increase the ability of the tissues to take load, you progressively overload and use eccentric activities. The problem that I have seen is that most clinicians focus on one or the other approach rather than both and then is left wondering why it did not work … go figure!
