Gait retraining or inserts for medial tibial stress syndrome

I have done a blog post and started a forum thread on the concept of when we have a runner with the medial tibial stress syndrome version of shin splints should we use an insert in the shoe or should we change the gait. The latest hypothesis on the cause of medial tibial stress syndrome is that it is due to too high bending moments through the tibia and there is some good circumstantial evidence supporting that but there certainly is a lack of clinical trials. To deal with the bending moments you can either place a wedge in the shoe or run with a wider base of gait. To me, it's six of one half a dozen of the other. Clinically both seem to work, so I guess it all comes down to individual preferences and experiences. A good clinician will avail both methods and not be swayed by the propaganda and rhetoric of one over the other.
